Full Arch Series: A High Esthetic Risk Patient Is a Highly Complex Case

There are many considerations when planning for a full arch case on implants. Even more so when we are talking about a high esthetic risk patient, which counts for about 80% of the full arch implant cases. 

It is difficult to create an exact protocol that fits all scenarios since every situation is different. Did the patient come in with existing implants and a denture? Does a patient want to switch from a regular denture to a screw retained restoration? Is he happy with the aesthetics of his current prosthesis? Are we starting from scratch with a patient?

The first step is to use the pictures we requested to the dentist according to our full arch on implant protocol. Then, we use Ricardo Mitrani’s evaluation of the esthetic risk by considering 3 factors: lips, tooth position and ridge to create the perfect smile with a 3D digital diagnostic wax up and also a 2D presentation mockup to present to the patient for a before and after reveal. 

From that, we will create a 3D printed surgical guide (stackable guide if necessary) and make recommendations for the material option (zirconia, Peek, acrylic on bar, double structure Peek/zirconia, etc.) We then get together in a virtual meeting and discuss as a team with the dentist and surgeon about the implant positioning, the management of the tissue, the material choice and lastly the restoration outcome.
